  • Covid-19: Fit note/ Sick note guidance and template       RMCA

We know that with measures such as social distancing and self isolation being put in place there are many more patients demanding fit notes/ Med3s. The current Government guidance for employers and businesses on coronavirus (covid-19) reiterates that medical evidence is not required for the first 7 days of sickness. It adds that after 7 days, employers may use their discretion around the need for medical evidence if an employee is staying at home.

New Patient Registration

We are happy to register new patients who have moved into our practice area. We do not discriminate on the basis of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical conditions.

Please be aware, that from the 29th of May 2019, our practice boundary has changed. We will only be accepting new patients from our reduced, St Peters Ward Catchment Area. All new patients need to fill in a GMS1 form and the Registration form, please note that for patients under 14 there is a different Registration form.
